Sorry about the accident. If this was the third dive of the day,She and her buddy would have perfored a minimum of 3 predive checks and theoretically numerous gauge checks throughout her dives. Open water students are taught never to dive with faulty or broken equipment and also to always check rental gear before they leave the dive shop with it, though that last part gets forgotten at times. So from what little you have presented again ends with self -responsibility I am soryy to tell you.
