We use a custom-made imprinted boards (we've actually got about a dozen of them) that list the diver's sign-in number, full name, and then three boxes for each dive. (We can out 4 dives on each board.)

On each dive in the first box, we note the time you jump off the boat. (This may or may approximate the time you descend.) The next box will indicate the time you put your feet back on the boat (as oppsoed to merely surfacing).

The third column is used, once we think we have all the divers on board, for a Visual Verification that all are actually present and accounted for. During the VV (we walk around and find everyone), every diver must be seen by the DM-in-charge whether or not they actually made the dive. Once all are accounted for, the DM-in-charge lets the captain know, and we can move to the next site. The same procedure is then repeated for all subsequent dives.

We feel using an actual "time off/time on" rather than just a slash is much more useful since it gives us an idea if someone's been out an inordinately long time. And if someone should complain of a bends-like pain, we can also refer to our board (along with their computer) to get an idea of dive profiles.
