Everybody makes mistakes because everybody is human, or so I have observed. Therefore everybody needs to be supervised at some level.

If someone has a track record that is really good, then they need very little supervision.

If somebody is on the other hand a beginner, they need a lot of supervision.

Never at any time does someone need no supervision.

A general headcount, of everyone on the boat, before weighing anchor, is the most general supervisory follow-up that I can think of, for the supervision of even the best D/M who is taking roll.

If the captain does the head count, or assigns another D/M to do it independently, that should work in confirming the roll call.

If that did not happen, then OOPS!
